Position:       Reentry Specialist

 

Purpose:       Reentry Specialist responsibilities are to provide pre- and post-release transitional services to help youth successfully reintegrate into their communities.

 

Reports to:   Sky Ranch Behavioral Services Executive Director

 

Geographic Area Served: Woodbury County, Iowa and surrounding communities within the 3rd Judicial District of Juvenile Court Services

 

Essential Job Functions

 

  • Support, guide, and mentor youth throughout the program with intensive case management built around evidence-based practices and strategies

  • Assist youth in increasing protective/prosocial factors, strengths, and coping mechanisms

  • Utilize ongoing assessment tools and individualized planning to deliver evidence-based interventions

  • Review youth progress and challenges in partnership with JCS

  • Provide wraparound services to youth to address basic needs, housing, education, etc.

  • Assist in systematic coordination to promote better outcomes for youth

  • Meet face-to-face with the youth while at Juvenile Detention Center, residential placement, and/or the community to engage youth in the programming

  • Collaborate with key stakeholders including juvenile court officers, family members, caregivers, JDC Service staff, and placement staff

  • Participate in YCPM (Youth Centered Planning Meeting) transition meetings and support implementation of the plan

  • Complete monthly progress reports to document and celebrate youth successes and update strategies to address challenges and barriers

  • Engage youth family members and caregivers in programming to promote success

  • Complete all documentation, narrative notes, case management plans, end-of-the-month reports, and discharge summaries

  • Collect, track, and report primary indicators of performance in conjunction with the external data evaluator


 

Education and Experience

 

  • Bachelor’s degree in human services or criminal justice related field preferred; consideration provided for education and work experience

  • Demonstrated experience and training in youth justice services and/or mental health, substance use disorder services, and mentoring.

  • Demonstrated experience in providing culturally responsive services to youth and families, primarily youth and families of color.

Pre-Employment Requirements

 

  • Valid driver’s license

  • Proof of vehicle insurance

  • Criminal, child abuse, and driving record background checks


 

Training Provided

 

  • Aggression Replacement Training (ART)

  • Adolescent Community Reinforcement Approach (A-CRA)

  • Mentors in Violence Prevention (MVP)

  • CBT Case Management utilizing YLS/CMI 2.0™

Salary Range

 

  $19.50 - $21.25 per hour (based on experience)
